FOR STUDENTS UNDER 18 YEARS OLD USING ASK DR. CASEY'S SERVICES

1. Purpose

Ask Dr. Casey offers college and career guidance services designed to help students make informed educational decisions. These services are advisory only and do not replace the role of school counselors or official academic advisors.

2. Acknowledgment of Scope

By signing this form, the student and parent/guardian understand that:

Dr. Casey Vaughan provides general educational advice, not guaranteed results or admissions outcomes.
Students are encouraged to confirm all recommendations with their school counselor or relevant institutions.
Ask Dr. Casey does not make final decisions regarding college admissions, scholarships, or academic placement.

3. Consent to Participate

I, the undersigned student, wish to participate in the services provided by Ask Dr. Casey.
I understand that participation is voluntary and that I can discontinue services at any time.

4. Parental/Guardian Consent

I, the undersigned parent/guardian, give permission for my child to participate in Ask Dr. Casey’s educational advising services.
I understand the nature of the services, agree to the limitations of liability, and release Dr. Casey Vaughan from any claims related to academic, college, or career outcomes.

5. Data Use & Communication

Information shared during sessions may be used to provide personalized guidance.
Personal data will be stored securely and will not be shared outside Ask Dr. Casey except as required by law.
